Co-Pilot Job in Burnaby, BC at BACI
The Burnaby Association for Community Inclusion (BACI) has been supporting people with disabilities for decades. We’ve learned a lot over those years and our learning has shaped the evolution of our services, our advocacy efforts and the way we help build inclusive communities.
The Stitched Service is a new way for people to move through services – it was designed using human-centered design principles, and is aimed at providing adaptable and well-coordinated supports for adults with a cognitive disability.
The Co-Pilot is the initial and ongoing primary point of contact for individuals and their support networks who are part of the Stitched Services at BACI. Through a creative and collaborative discovery process and by building a shared profile with the individual, the co-pilot works to, with the individual, develop a plan of action and makes recommendations about the best mix of services.
With extensive knowledge of the different BACI services, the Co-Pilot supports seamless movement through different areas of the organization and coordinates collaborative efforts between departments that are working towards a variety of coordinated outcomes. The Co-Pilot recognizes and capitalizes on emergent opportunities to support individuals in achieving the best outcomes, and can identify opportunities to leverage both staff and community resources in support of these outcomes. The co-pilot provides ongoing case management support and helps individuals to navigate complexity.
